You can refer someone for managed live-in care by using our short online referral form, which asks only for the minimum details we need to triage, or through your organisation's own approved secure route. We consider live-in care and substantial packages across England where we can staff and manage them safely, with Cheshire West and Chester as our home area. A member of our team reviews every referral, and we agree a secure way to share anything sensitive.

- We agree a secure route for any further information.
- We assess the person, at home or on the ward.

- We keep you updated and review the care as needs change.
We never promise a start date before assessing the person's needs.
Keeping information safe
Please don't send medical records, discharge summaries, care plans, medication records or detailed risk assessments through our website. Our online form asks only for the minimum information needed to triage, and email notifications contain no health information.
Frequently asked questions
What information do you need for a first referral?
Only what we need to triage: your contact details and organisation, the person's general location (postcode district), the type of support needed and the expected start or discharge date. We then call you and agree a secure way to share anything else.
Do you accept local authority or NHS-funded packages?
Please call us to discuss the package. We consider each referral individually, depending on the person's needs and whether we can staff and manage the care safely.
Can you assess someone on a hospital ward?
Yes, where it is safe and the ward agrees. Assessing before discharge helps plan care to start when the person gets home. See our hospital discharge page.
Do you provide nursing care?
No. We provide personal care and support. Any delegated healthcare tasks are undertaken only where appropriate to the individual's care plan and where the carer has received the necessary training, competency assessment and professional delegation or oversight where required.
